Subject: One time wonder?

I've had this song running in the back of my mind for years and never thought to search it out until a friend mentioned it a few days ago.  Anyway, the song was from the early 70's I believe and I don't remember the lyrics but I do remember the subject.  It was about a guy who got a bill for something and there was an IBM punchcard that said "Do not fold, spindle, or mutilate" on it.  So the guy folded it and spindled it and, this is the only part of the lyrics that I can remember, "punched some holes where there weren't none before".  He sent it back in and received a letter of apology and that they make a mistake and owed him money.  Any help or clues would be very welcome.  Thanks ;D
